Our Community Choice Guide Program has been helping people living in residential settings and nursing homes relocate back into their communities. The services we provide aid individuals transitioning to different levels of care with relocation support, stabilization services, and safe community integration.

Our Community Choice Guide Program has been helping people living in residential settings and nursing homes relocate back into their communities. The services we provide aid individuals transitioning to different levels of care with relocation support, stabilization services, and safe community integration.

Program Overview...

Our staff provides intensive one on one relocation support for individuals moving from one setting to another, such as institutional settings to lesser levels of care (including independent living), to find services and get everything set up for a safe transition to home and in the community.

As contracted providers, our Community Choice Guides (CCGs) work with local housing authorities and other community resources to locate and arrange appropriate, accessible housing for eligible clients as authorized in the client’s plan of care. CCGs act as a liaison with and for the client, assisting with tasks such as…

  • Filling out forms or applications and finding helpful community resources. Community-based assessments.
  • Arranging physical moves and ensuring medical resources are available and met.
  • Setting up less formal or atypical daily activities.
  • Arranging for necessary relocation expenses; examples of expenses for qualifying individuals can include…
    • — Fees or deposits for utilities or services necessary for an individual’s health and safety such as pest eradication and one-time cleaning/allergen control prior to occupancy.
      — Security deposits.
      — Household furnishings.
      — Moving expenses.

Program Components...

Our CCGs provide pre-transition and post-stabilization services as authorized, defined, and directed by the Case Manager. 

Transitional Services…

  • Orient clients to new settings such as bus routes, grocery shopping, library visits, senior center activities, etc.
  • Assist with finding/relocating to another community setting, if needed for stabilization.
  • Take clients to the Social Security office to update the status of their benefits.
  • Shop for authorized items.

Stabilization Services

  • Coordinate with client/family, community partners, care staff, housing, medical and legal personnel, caregivers, and other supports.
  • Assist in obtaining ID and filling out applications.
  • Attend Care Conferences in SNFs or hospitals.
  • Visit housing options with/for the client.
  • Assist with moving arrangements.
